PC/Monitor Crashes When Exiting Games

For the past couple of weeks, I've been having this issue where when I exit from a game, my monitor 'disconnects' and sometimes the fans on my pc also ramp up to 100%, although that doesn't happen too often. Usually though it just says 'no input detected' on my monitor then it goes black. When I try restarting my PC, if I do not first turn off my monitor and restart it too, it will still say no signal when I restart the PC.

I can still hear audio etc from my pc and talk on skype after the monitor disconnects.

I have tried looking at my temperatures and fan speeds while playing games and nothing is abnormal. System doesn't run hot or anything. I can play the same game for 4 hours straight without a 'crash' but as soon as I click on exit or alt-tab back to desktop it will crash. (Not 100% of the time on alt-tabbing, however exiting games results in a crash about 95% of the time)

I have since tried upgrading from windows 8.1 to windows 10 and reinstalled all drivers in the process. Problem persists although seems to be occurring slightly less often.
