Strange New Problem (Perhaps OS issue?)

So about a week ago I get on my PC before I go to work and notice that it was extremely laggy and slow. So slow in fact that I was never able to make it past the login welcome screen. I tried restarting my PC multiple time, but to no avail. When I got home from work I tried getting on again and was eventually able to get on but everything was extremely laggy and slow. Any time I tried to run a program it would freeze up my computer and I couldn't do anything about it and would have to restart my computer again. Eventually, after many restarts, I was able to get my computer to do basic things, so I ran Malwarebytes and Avast(multiple times), neither of which found anything harmful. Another thing I noticed was that whenever I tried to run a program that refused to load my "disk usage" in task manager would shoot up to 100% and then the program would fail to load up (I tried mutliple programs like Chrome, Steam, Afterburner, File Explorer). Thinking that it might have been a HDD problem I decided to run HDtune to check for hard drive defectives. I think it ended up coming up with .1% bad parts, which I assumed was ok(honestly have no idea). After not finding any solutions I decided to try a system restore in hopes that whatever happened would be fixed by loading up from before my issues. Unfortunately, my problems were not solved. The very next day however I noticed that my PC was working just fine. The PC loaded up as fast as ever, Chrome opened, and I was able to function pretty much normally. Except even after this problem mostly fixed itself I'm still noticing some weird things. First of all, every couple of times I try to login, my PC seems to take a little longer than normal(it's usually instant). Also, Chrome is extremely choppy when scrolling now. So choppy that it's hard to even use sometimes, and what's weird is that this problem does not copy over into MS Edge which works just fine. Also, for some reason my computer's sleep mode setting keeps getting turned off. Whenever I first had my problems I noticed that my computer wasn't going into sleep mode and when I checked the settings I noticed that they were disabled, so I renenabled them. But once more my setting have disabled themselves again for no reason. Another problem that may or may not be related is the fact that the game "Squad" is now very laggy for me which it wasn't before. And another game "Verdun" made my r9 390 reach 94C which I've also never had happen before because I have pretty good airflow. If you have any idea what could be causing my computer to be acting up I would greatly appreciate your input. I honestly have no idea what could be causing me issues, which is why I didn't really know which thread to put this in.

Unfortunately, I'm afraid it is! :( I'd strongly recommend you to get in touch with our Customer Support either by phone or e-mail and let them know about the failed Extended test from the DLG tool. The WD Black HDD should still be covered by the warranty which means that you can send your RMA request.
Our customer support will give you more details about the product replacement procedure.

I'd recommend you to backup any important data from the rig somewhere off-site first and then proceed with the troubleshooting.
I'd start by checking the connections inside the PC case and make sure you have plugged everything properly. If you have a spare SATA cable, I'd advise you to swap it with the one currently connecting the WD Black and also plug the HDD to a different SATA port on the mobo.
If the performance doesn't improve after that, you should definitely run the QUICK and EXTENDED tests from our WD Data LifeGuard Diagnostics for Windows. The utility will help us determine the health and SMART status of the HDD.
If things don't get better, try plugging the WD Black to a different computer and monitor its behavior there.

Thank you. Alright, so I downloaded and ran both the Extended and Quick scans which both came back negative. I also tried to repair after the Extended scan which also failed. After this I decided to switch sata cables and plug it into a new port (I had actually tried this when my problems first arose). After my computer booted (it did the slow boot thing) I ran the Quick scan and it came back positive this time. I'll run the Extended scan later and report back my results.

Alright, so I just got home from work and checked on my computer and the second Extended scan has failed as well. What does this mean? Is my HDD faulty?
