This is a bit odd if it happens with all computer you try the flash drive with. Do you have any files you need to recover from it? If "no", then you could try reformatting it either by using Disk Management, CMD, etc. or if that doesn't work, by download the manufacturer's formatting tool (most SD card and USB flash drive manufacturers offer such tools for their devices) and use it, to see if it fixes things up.
Unfortunately if nothing proves useful, it's very likely that the flash might have failed. If this is the case, you can try to RMA it (if it's still udner warranty) or get a new one.
