It is an old GPU nVidia geForce 8800 GT from XFX and it is stuck at 669 MHz clock speed
, according to Evga precision, even when gaming. I have opened it, cleaned it, and applied new thermal paste recently. But the problem didn't happen after that. If I manually raise the clock speed to ~700 it does crash
