Should I Upgrade my Graphics Card or CPU?

I've been playing Dark Souls 3 recently and I've noticed that the game isn't running as well as I thought it would, and it's kind of got me thinking of buying a new graphics card. I have an R7 260x and I know those are pretty decent, but I think it's time I could use a little upgrade.

Looking over a Game-Debate and seeing if I could run Dark Souls 3, I also noticed how my CPU was 25% below the requirements, while my GPU was about 10% above the requirements. So that also got me thinking of buying a new CPU (I have an FX-6300).

The problem is, however, is that I'm kind of on a low budget, with about $150 on hand, maybe another $50-100 from a friend, and just recently put about about another $150 worth of items online. So overall I will have $200-400 to spend (how much money not confirmed at the moment). And I'm very indecisive on what I want to buy right now.

Right now, the GTX 950, 960, and 970 are on my mind, and I'm not too sure what I want to get for a CPU if I choose to get a CPU. Perhaps I could get the FX-8350 or 8370, but I'll leave that up to you guys. I'm definitely not looking to run the best games on the highest settings, I just want to be able to reliably run the more high-end games. Currently I can run games like GTA V (decently) on medium settings.

And ONE more problem with buying a new GPU or CPU is that I have a pretty bad power supply (at least that's what everyone says). I have an AL-D500EXP from Allied that has a max wattage of 500W. So I'll most likely need to get an upgrade on that as well. I'm a little new on the whole upgrading PSUs and GPUs and all that, so any help will be nice.

Long story short:

Should I upgrade my CPU or GPU? Both?
Will I need a new PSU?
What can I get that's good on a $200-400 budget?
Should I not even upgrade and just save up?

Again I have an FX-6300 Six-Core 3.50 GHz for my CPU and a R7-260x 2GB for my GPU. I just want to be able run most games's recommended requirements if possible. Any suggestions for what I should buy would be nice.

Yeah I'm considering getting the 970. What about a new power supply?

So if my PSU has a +12V1 with 15 Amps and a +12V2 with 16 Amps, would those amps combine to meet the requirements? Or will I have to have a single 12 volt rail with at least 28 amps?
