Computer freezes and monitor loses signal

Hello, my problem is that when i play dark souls 3 (and it happened once in fallout 4, its happened 3 times in the past 3 days in DS3, about once every day, i only play ds3 for like 30 mins everyday, doesnt happen on any other game no matter how long i play it) there are moments where the game freezes and my monitor loses signal. what is weird is that the game runs very smooth, the freeze is just random, something else is that it only happens when I'm on teamspeak, when it freezes and loses signal I can still talk and hear team speak very well. Another issue which might be related is when i startup my computer or sometimes at random my monitor goes black and then goes back after a second and it happens a lot in one minute and goes on for like 2 minutes. I have stress tested my gpu using furmark for 30 mins and didnt get any problems at all. Sometimes it freezes and i manage to alt tab in time and close the app, then i get an error saying amd driver has stopped responding and has recovered. One solution that i thought of was updating the mobo drivers since all the graphics card drivers were up to date, after i updated the mobo drivers everything was perfect , no errors, but then it came back two days later. This never happens in the windows login screen at all.

My specs are: intel i3-6100 evga 500w 80+ certified r9 380 4gb Gigabyte lga 1151 h110 micro atx 8gb ram
My os is on a hard drive that i got from a old computer of mines, i have another hard drive which is brand new and i only use for games
 
Solution
It sounds like you have got a bad card (which unfortunately happens). I would RMA it for a new one and tell them it never worked right out of the package.

This is definitely a GPU issue and based off your statement of still being able to hear others in your teamspeak room, I'm willing to bet its a physical issue. Has it done this since day one or is this something recent? Before this happened did you by chance update the drivers?

This has been a day one issue, and i updated the drivers the first day
