GA-Z170-D3H showing multiple UEFI partitions for the same drives in BIOS and Boot Menu?

I recently bought the GA-Z170-D3H motherboard. These are the drives connected to the motherboard:

SSD: Crucial MX100 256GB
HDD: WD Blue 1TB
USB: Kingston 8GB 2.0

Whenever I boot my PC and go to the Boot Menu (F12) it shows multiple UEFI partitions for my SSD, multiple UEFI partitions for my USB and for my HDD. The worst thing is that it is completely random. Sometimes there will be 2 UEFI partitions shown for the SSD, 3 for the USB, and maybe none for the HDD. Also, sometimes there are just blank selections (you can click on them, but they are empty, no drive name). Also sometimes there will be selections with just the drive name (e.g Crucial MX100 256GB), and sometimes not.

I can click on any of those UEFI partitions and it will boot to the corresponding OS (e.g. I have 5 UEFI partitions displayed for my SSD, I can click on any of them and they all work).

All the symptoms also happen in the BIOS under the 'Boot Override' menu.
I've tried changing SATA ports and USB ports multiple times and it didn't help.

I should also mention that all drivers work absolutely flawless when I boot into an OS.

What's the problem here? Is it a problem with my MOBO? Thanks.

1| Can you please pass on your full system's specs inclusive of your OS and connected peripherals?
2| The issue could be pinned down to your motherboard or your installation of the OS. Have you made sure your BIOS is up to date while the firmware for your Crucial SSD is up to par?
3| Does the issue crop up when you have only your BOOT drive in place connected off the first SATA port?
