Did I caused damaged to my GPU because of overclocking?

I have a r9 380 nitro and I've been trying to figure out the overclock thing for about two weeks now. Thing is, I never could reach a stable OC so I started over again and I noticed, arctifacts might be appearing at lower core clock speeds than before. So my question is, Did I fucked up and permanently damaged my GPU? I never pushed it too hard (I never surpassed what other people had done with this card according to different setting configurations I found on internet), but I could never make it stop crashing either so I don't know. It's not a big deal either, if I leave it at stock clocks it's all fine, but Im neurotic and paranoic and I need to know what's the probability of having damaged the card.

PD: Don't worry about OC anymore, I'll just stick to stock clocks, I dont want to keep risking my GPU.

Edit: I should add that i did indeed messed around with voltages, but only up to +50mv and my PSU is a 750w with 87% efficiency so i dont really think I messed up in that regard... or did I?

Idk if that small a voltage boost would affect your card, but I once burnt out a bit of ram on an old ati card by adjusting voltage without knowing what it would do, it would appear in games as a rectangular box instead of a part of a vehicle for example, it was always noticeable no mater what game you played.

Msi Afterburner allows up to +75mv so I would think that to be the upper safe range, I run +10mv on my cards with stock clocks.

Stock clocks are generally set within a few mhz of max clock without artifacting under stress, I'm not familiar with amd overclocking but nvidia lock the voltage to prevent damaging the card with a higher clock. I have had great success overclocking for benchmarking but generally you don't want to run the card overclocked for gaming unless (in the case of the 700 series cards for example) you can get a stable ram overclock or in the case of the 600 and 900 series where you can get a stable gpu overclock which will run fine while gaming.
