Monitor Distortion (GPU issue?)

Hey, everyone.

To begin, I have a custom PC built in 2013.
CPU: Intel i5 - 4570 @ 3.20GHz
GPU: ASUS GeForce GTX 770 DirectCU II
Mobo: Asus H87-Plus
Monitor: BenQ GL2450H
RAM: 8GB Gskill Sniper (CL9)
PSU: Antec 750w High Current Pro
1TB HDD, 2 TB HDD, 120GB SSD
Fans: - 20mm Front, 20mm Rear, 2 x 40mm Top
OS: Windows 7 Home

Over the past 3 days, while playing Guild Wars 2 my monitor has gone black (usually lasts 3-5 seconds) before returning to normal. These black screens would happen one or two at a time, then not for at least another hour (if at all). Previously, I had an issue where my monitor would go black and I would get a "No Signal Detected" and it ended up being that one of the cords inside my case had been preventing one of my 2 GPU fans from spinning. I fixed this and went symptom free for about 6 months. Similar symptoms has me guessing it might be the GPU again, however there is no blockage, fans are running smoothly and no odd sounds or vibrating coming from the GPU. It is sitting correctly. I run MSI Afterburner while I play Guild Wars 2, so my first instinct after the blackscreen was to check my Afterburner graphs. CPU Temp sat around 60 degrees and GPU around 67. Utilization was normal, no apparent issues with RAM or hardware. Nothing out of place as far as I could tell. I ran memory diagnostics and no problems with RAM were detected. I've yet to run any GPU tests because of the following:

Today, (while playing Guild Wars for approx 1hr 30m - previous times ive played 6+ hours with no issues) my screen went black, but this time it stayed black. It had gone black for a few seconds before this, which led me to quickly check my Afterburner. Everything seemed normal. Anyways, after it stayed black, I forced it to shut down. My GPU was hot to the touch, but not so much that it would burn me or anything like that. When I restarted it.... The result wasn't pretty to say the least. It led to this: (pictures taken approx 3 seconds apart. Began with lines, then began to distort quite heavily.

So is my Afterburner lying to me and my GPU is overheating? Or is my GPU just starting to die? I purchased it brand new in 2013 if that's of any relevance. Thanks in advance to anyone who read all that!
 
Solution
It has to be hardware related since drivers are not loaded yet on the windows boot screens and you are having issues. Could be psu or mobo too. It's hard to tell other than switching components which is not possible with most people.

Currently on Version 365.10 (Released May 2, 2016). Note that I had installed the latest driver AFTER the problem began. Updating driver had no effect, unfortunately.

I ran a fullscreen Furmark benchmark test for 20 minutes straight with no crashes or blackscreens. (Fans on max). GPU reached a maximum of 82 degrees, but no crashes, black screens, or frame loss.
