I have a dell optiplex 745 sff with a core 2 quad Q6600 what is the best GPU to match for under £150 because its small form factor I'm limited to single slot cards I was thinking about getting a pci-e raiser cable are they worth it??

With the 275W PSU, you will be limited, anyway. The GT 730 DDR5 64 bit is the fastest card recommended for that system.

(I assume it comes with a L-P bracket)

But I have been using a GTX 750 Ti in my Optiplex 780 SFF for quite awhile with no problems. It has the same 275W PSU specs as yours, I believe. This MSI is a single slot, low profile GTX 750 Ti that should fit if you want to go that route: https://us.msi.com/Graphics-card/N750-Ti-2GD5TLP.html#hero-overview

The GT 730 would be safest with that small PSU. But if you are like me and want to take the risk, the single slot GTX 750 Ti I listed above will work. My 790 SFF is designed a bit differently allowing me to use a double width card. I chose the Gigabyte GTX 750 Ti:
