Difficulty with ASRock Extreme 4 (z87) and 290x

I'm running into some difficulty with my computer. It runs perfectly fine on integrated graphics. However, whenever I attempt to install and use my GPU (Sapphire 290x Tri-X), I get the following post codes:

4f 78 b2 9c a2

Likely 19 times out of 20, my computer won't start, the fans go full speed, and I get these post codes, but the few times that it does start, it runs fine. What could cause this?

ASRock Extreme4 z87
Sapphire 290x
Rosewill 750M

I also get black screens (computer just instantly shuts off) on videos and other 2D tasks, but there are no issues gaming. Like I said, the machine runs fine without the vid card, so I'm not having issues with other components and I've RMA'd my card 4 times due to this issue. Replacements show the same symptoms.

4F = Memory problems
A2 = IDE or SATA problem
78 = Chipset initialization error

I suggest trying one ram stick at a time or even swap it out with another machine if possible. If you can try another video card, do so.

I've tried it with another video card (a 7950) and it works fine. However, I'm unsure why the 290x would still be having issues, as this is my 4th replacement with the same problems. Surely I wouldn't get 4 defective cards in a row, right?

Unfortunately, the card is beyond the return period, so it can only be RMA'd. The latest one hasn't had any issues with the black screens (Yet... I've only had it 3 days now), but I'm still getting the BIOS issues at startup.
