No video and GPU fan at max speed, disconnect/reconnect 12v helps for some time

I just got a new PC build and while playing a game suddenly the screen became black and GPU fan started spinning at max speed.

The PC didn't reboot but didn't response as well, so I had to reboot it with reboot button.

It started, but when I started the game that happened again.

I restarted again and ran 3D Mark. Every time I started it everything happened again (black screen, max GPU fan speed).

At some moment it stopped booting sometimes.
- No video
- CPU fan doesn't start

When it booted it showed low 12v voltage (10.7 - 11.2) in BIOS.

I've disconnected 12v power from motherboard and then reconnected it. Everything worked like a charm for 2 weeks (could finish 3D Mark GPU test, EVGA OC Scanner tests and etc.). The voltage in BIOS and HWiNFO 64 was around 12.0v.

Today it happened again. I've tried reconnecting 12v from motherboard and now everything is fine again.

What could be the issue? PSU? Motherboard? GPU?

EVGA GeForce GTX 980 Ti
Z170I GAMING PRO AC
SST-SX600-G 600W
