Monitor randomly displays "Out of Input Range" even after I set the resolution and refresh rate to native settings.

THE FIX NO LONGER WORKS

Unplugging does nothing anymore, the error is back again.
Please help me, I'm tearing my hair out at this point.





The Problem

So just recently my monitor started randomly displaying "Out of Range" whilst in-game for no reason. I can't exit out of this 'error' unless I restart my PC. I can't CTRL+ALT+DEL, CTRL+SHIFT+ESC, or WINDOWS+D out of it.

Examples
I was playing Team Fortress 2 and after an hour, my monitor gave me the out of range error. Also when I try playing Bioshock, everything works up until the cut-scene ends, and I get the error. I cold boot, and in the same resolution launch League of Legends, it works completely fine. However, this error will never happen on my desktop. Before I took out my GPU and RAM, the error would happen AFTER I logged in, or sometimes not. After troubleshooting for a couple hours, I restarted my PC and I heard THREE loud beeps. Thinking it was a RAM issue, I took out the GPU and RAM, blew on their lanes to get rid of the dust, put it back in, and now the error ONLY happens in-game.

What I've Tried

• Setting refresh rate and resolution to my monitor's native settings, because after all, Out of Range means it's a resolution/refresh rate my monitor can't handle
• Manually setting resolution in the game's .ini files
• Using Steam's launch options
• Reinstalling graphics drivers
• Taking out RAM and GPU and making sure there is no dust, then putting them back in again
• Switching monitors during the glitch (the other monitor displays the same thing)
• Using the lowest resolution or 1024x768 (still happens). Weirdly enough, 1366x768 resolution in Bioshock gives me the error, but 800x600 doesn't. Some games are 100% likely give me the error, such as Bioshock, but in the same resolution, League of Legends works. Some games it randomly happens, like TF2
• Manually forcing a resolution in Nvidia control panel
• Verifying integrity of cache for games that don't work
• Using menu key on my monitor to try and reset the resolution. For some reason, none of the buttons (except the off button) work on either of the monitors I tried when the "Out of Range" error occurs.

Unfortunately, I have no clue how to figure out which monitor I have, but I know it's nothing special, is very old, and has a refresh rate of 60 and a resolution of 1440 x 900.

I reinstalled my graphics driver and installed an older version (364.51) and I didn't instantly get the error, it took a couple minutes.

My monitor has randomly started displaying "No Signal Input" throughout the day.

nope, sometimes when I'm logging in, after the windows boot logo, or after I enter my password, I'll get the error.

I switched the cables and ran it on the integrated graphics. (No error with integrated graphics) How do I reset mobo cmos and jumper setttings, and what do they do?

 
ok...so the issue is with the gpu then...can u try this gpu in another system or try another card in this system, and check whether the issue follows the card???

there is a cmos coin cell of silver colour which u have to take out and re-seat after a gud 10-15 mins to drain out the battery...this resets the mobo sttings completely...the jumper pins are either 2pin or 4pin and can be anywhere on the board...u need to refer to ur motherboard manual for this...
but as we are narrowing down the issue to the card...lest leave this part for the time being...

oh jesus, I probably threw away my mobo guide. Oh well. Sadly I dont have any other system or gpu. I'm ordering a new generic hd monitor, ill try that before any crazy cmos mobo stuff.

 
as u have already switched the monitor before with the same result, i dont think tht will work...u shd rather take a harder look at the gpu as the problem only persists with the dedicated gpu and not the integrated one...tht itself suggests its a gpu card issue...

hey, big news. So whenever I get the error ingame, I get a no signal input error as soon as I boot up my pc. Just a second ago, I tried booting up my pc, and heard 3 loud beeps. I restarted and I heard nothing and my pc worked fine (until I got the error again) I then tried plugging my monitor into the mobo, and for some reason it says no signal input. I don't think its an error, I think my vga header on my mobo is broken or something. I'm beginning to become suspicious of my PSU, because it is notoriously shitty and has terrible ratings. I'm going to try and get a better rated more powerful PSU and test it out.

Hey, I think I fixed the problem once again by taking apart and rebuilding my computer
