Graphic Card for my Core 2 duo + Intel dp45sg

Hi guys, this is my first time posting here. I am not very familiar with graphic cards so I'm trying to get help from you guys.

Procie: Intel Core2 Duo E7500 Wolfdale Dual-Core 2.93 GHz
Mobo: Intel DP45SG
Ram: 2gb DDR3 1333MHz (I can add 2gb more)
GPU: ATi Radeon HD 4600 Series
PSU: Generic 600w

I can play dota2 at low settings. What video card can I get so that I can play at Medium to High settings? Also, I want to play like fallout 4 and GTA5. Whats the cheapest or most cost-effective Video Card out there?

Yes, you can. That PSU looks like a cheap, low quality one, get it replaced if you can. For GTA 5, you need 8GB ram to play without stuttering. If you can get a GTX 750Ti, or if not, get a GT 730 with 2GB GDDR5 memory ;)

The R7 250 1GB DDR5 or GTX 750Ti 2GB DDR5 (if youre on tight budget options, pick R7) will play GTA V in your Core 2 Duo in medium settings, you might need to upgrade your RAM to at least 6GB but i recommend you for 8GB system memory. 4GB won't be enough as you can see today's gaming average memory is still 8GB. Just to make sure if you're going to play another games, you don't need extra memory.
