Help. PC keeps freezing.

Alright I can't seem to find a correct category for this so I'll post this here and if it's at the wrong place please move it for me thanks.

So I just purchased a Sapphire Nitro R9 380X 4GB GDDR5 graphics card on the 10th of june. Installed it and everything was fine, I could play Dark Souls 3, DA:I, and practically anything on my desktop.

And just today, everytime I turn on my PC the screen just freezes there. There are a couple of things I noticed:

1. If windows was already booted and the screen freezes, well it just freeze there. Nothing I press or click makes any difference to it.

2. Sometimes the screen freezes for a few minutes then my monitor just blacks out and the power light changes to orange. It doesn't display anything like "No signal" or "Cable not connected". Just a frozen black screen.

3. If scenario 2 happens and when I reboot my PC, the monitor wont come alive. It just stays the same at the black frozen screen and that orange power light. If I wait long enough, I can hear my windows booting but no display at all.

4. If scenario 3 happens but it just doesn't boot and I restarted my PC a few too many times the CPU sounds off 5 short beeps. The screen still stays frozen at the black screen.

5. Sometimes after I reboot my PC, the PC refuses to read my GPU and device manager says my display adapter is the on-board GPU even though my cables are connected to the Sapphire card.

6. Everything is fine and normal if I use the on-board VGA or my old GPU. No freezing screens.

My specs:
PSU: SmartSE 630W
MOBO: ASRock H61M- HVGS
GPU: Sapphire Nitro R9 380X
RAM: 16GB
OS: Win 7 Professional x64

I kinda forgot my processor.. hope somebody can give me some suggestions on what to do. Had been trying to fix this for the past 12 hours but to no avail.
