Gigabyte GTX 980 only one displayport works

So I've been using my computer with a Gigabyte GTX 980 and It has been working perfectly fine up until recently when I turned the computer on and nothing came up on the screen.
Through some testing I've discovered that only one of the display port outputs work, the other two boot to the windows loading screen then either display nothing or blurry lines across the top of the screen which are slightly different every time I boot up the computer. I've tested it with different cables and ports so I'm fairly sure it is not the monitor or the cable that is to fault.

I'm using a Gigabyte GTX 980 G1 Gaming with an ASUS PB-8QR

Any ideas on what to do?

maybe you still have a warranty? I try to see if you can still rma that card, also it could be something wrong with a driver, I would uninstall all of your video drivers and reinstall them gpu boss has a program that makes sure all of the video drivers can be deleted.
