GTX 960 lod problems

On every single one of my games models keep popping up. Any fixes?
The specs of my PC are as follows:
ASUS GTX 960 Mini 2GB
AMD FX-6300
8GB of RAM
1 TB HDD
Windows 10
400w PSU

Models in the distance are just popping up for example: "Tree leaves in the distance pop out of nowhere"

My PSU is just a generic 400w PSU and I reinstalled the drivers and it didn't work

The tree in the middle has no leaves but when I get up close they pop up.

seems like a rendering problem, are you running the game in 32 bit or 64 bit?
