White boxes artifacts all over the screen

hi,
I have Nvidia GTX 680 and using windows 10.
Randomly, these white boxes appear all over the screen after the OS boots. This problem first occurred 2-3 weeks ago 3 times, i did nothing just restart my computer over and over and it didn't appear again until today and now its not going away. It appears anytime randomly after the OS boots up. White boxes appear all over the screen for a while and then either the system restarts or sometimes it gives different BSOD messages mostly related to nvlddmkm.sys.

I read many forums related to this and followed some of them.

1. I updated graphic driver to the latest 368.39 as well as windows is up to date but it still occurred.
2. I rolled back my graphic driver to 368.22 and the problem occurred again. I rolled back to much earlier version 359.00, still the same.
I used DDU uninstaller and did clean installation everytime.

In safe mode and even without any graphic driver installed, the system works perfectly without any artifacts but as soon as I install any nvidia driver the problem occurs again.

Is this the problem with my graphic card or the nvidia driver issues ?

my pc specs are :
2TB hardisk
16gb ram
intel 3930k processor
nvidia gtx680
gigabyte g1.assassin 2

Any help will be greatly appreciated.
 
Solution
Most likely issue here is a bad video card. Test it in another system that can run it. It may also be a power supply issue.

If you want to rule out a driver issue, get a spare drive, install Windows clean on that with new drivers setup and see how the system runs.

Checking for a BIOS update is also a good idea.

I'd say 75% it's a bad card though.

Tried fresh installation of windows 8 and the problem still occurs. I guess its the problem with the card itself.
