*UPDATED ANSWER*
Actually, I've found out my HTC Vive HDMI port and my monitor's HDMI port were "feuding" in a way and confusing either said monitor or Windows 10 on wake-up. I'm not sure why, but one of them just doesn't like having two HDMI ports plugged in. Perhaps it could even be the HTC Vive's HDMI port attempting to take priority before the monitor turns on.
Anyway, I keep the Vive HDMI port unplugged until I want to play VR, and I haven't had the problem since. This seems to be the culprit. Also, thank you for the best answer, even though the previous answer wasn't the true answer.
I hope this updated, current answer helps more people than my previous one. If you have two HDMI cords plugged in for any device, try keeping the one not connected to your monitor unplugged when waking up from sleep.



*OLD ANSWER*
Most of the time after I wake my PC running Windows 10 from sleep, my display is locked to a 30 refresh rate. It is usually 60, and can be reverted to 60 after a reboot, but I'd like to not have to do this every time. Running a Titan Black with the latest drivers and an i7 processor. I already tried reinstalling the gpu drivers and checked for Windows updates. The display is an LG Electronics UM95 Ultrawide screen (3440x1440). Supports 60 refresh rate natively and it's connected directly to my Titan Black via DisplayPort, the only DP available on my build.

Well just turn off your PC everytime you are done with it and problem is solved. Quick boot takes 10 seconds so by the time you settle in your chair your PC is ready to go.
Anyway try using different VGA out port. See if that helps.
 
It will depend on what display you have, and how you have it connected.
Can you state the monitor, it`s size in screen, and how it is connected.

A 30Hz is something you would or may get if you were using a large LCD tv.
Where 30 Hz would be a Interlaced screen resolution. 1920 x 1080 i
Over a 60hz progressive screen resolution of 1920 x 1080 p

It would suggest that the tv is not in sync with a 60hz signal or it is slightly to high for the mode to switch to a progressive mode.
There should be an option for 59hz that may resolve the problem from sleep of the system in relation to the screen mode displayed.
But it is only available for set screen resolutions not all that the card can display depending on the monitor or tv used.

@maxinexus I would do this, but I use a large number of programs daily and I find it a pain to open each one up and coming to realize half the programs don't save the current state they are in, etc. Too much of a chore, but I figured I'd come here and find a fix before resorting to that, which will probably be my last resort.

I'd rather not use HDMI instead, for the DisplayPort provides better color, more information transfer and etc.

I assume you have the latest nvidia driver :) I run win10Pro on Samsung 28UHD monitor and GTX970. I use DP port as well. DP gives 60Hz. HDMI supports only 30. When I got the display I had bit of color issues but changed the DP cable and since then I had no issues. Maybe that will work for you as well. Although, your case sounds more like software problem. It could be one of the programs you are running. Try elimination method and see if closing one program would solves the issue.
