Do you mean you formatted the HDD did a clean install, it worked for a day, then back to the same issue?... If so, and you did not connect a removable media, it would mean a hardware issue.. I've had HDDs that had issues.. I could format or wipe and install Windows on normally but days or weeks later went back to the same issues. Problem is your HDD checks 100% normal so it's hard to tell what really goes on. I'd suggest you wipe the HDD from the Hiren's Boot CD/USB, and before initiating it which would mean creating partition tables, partitions and file system, remove it from the computer and reset the BIOS for an hour and only then connect the HDD and initiate it. This would take care of possible partition table or BIOS infections.
To wipe the HDD...
a) delete partitions,
b) format the HDD,
c) wipe it,
d) format it and remove it.
And before installing windows again, reset the BIOS to make sure a possible virus can't be saved anywhere in the computer.
1. remove power from the computer
2. remove the BIOS battery
3. move the BIOS jumper to pins 2 & 3
4. press and hold the power button for a minute
5. wait an hour, and move the jumper back to pins 1 and 2, drop the battery in
6. connect the HDD and AC power
7. boot to the Windows installer, format and partition the HDD, and install Windows.
Can't see how after doing this routine a virus could still remain intact... unless it comes from an external source. Somewhere like removable media or another computer from where you normally transfer files via homenetwork or removable media. So, for a reasonable period of time after installing Windows, make sure the computer doesn't come into contact with a possible source of infection.
This failing would mean it's either the motherboard or HDD, or the Windows installer if it's not an original DVD or built from an ISO that was downloaded from a reliable source.
