Terrible stuttering in games and generally bad performance

So i have decent set up with following components
FX-8350
GTX 960 4gb
Asrock 960 gc-gs fx
Corsair vs650
16gb (2x8gb) memory
2 hard drives (one for programs and other for media etc.)
and 120gb ssd for OS

For years this setup has performed very well but in the past year it has been getting slower and slower. Now using google chrome and such is painful. What is suspect to cause this is my hard drive that stores all the programs. Few months ago i had to get new hard drive because the old one wasn't enough (500gb) so i upgraded to 2tb and soon it died (more bad sectors than healthy ones) so i got a new one, different brand and better quality. aaaand nothing has changed, still terrible stuttering everywhere. For example while playing bf 4, i'm the last one to pass the loading screen, in game i get decent fps but there is terrible stuttering from 60fps to 10 and below. My components aren't overheated at all. Can psu cause this kind of symptoms if faulty, or what could cause this? I've tried all kinds of things and nothing so far has helped at all.

A 120gb ssd is too small for the "C" drive these days. Many things want to go on the "C" drive.
If a ssd gets to 90% full, it will slow down and lose performance.

This is particularly likely if your ssd is not one of the better ones like Samsung or Intel.

Another possibility is that the motherboard is not really suitable for a FX-8 and is deteriorating.

Hi,

Are all your drivers up to date, especially the graphics one? Have you tried checking if you do not have too many resource intensive/useless applications running in background while playing? Are your games installed on your SSD or HDD?
Have you tried a new and clean install to see if you have the same behavior?

I've done clean install 5 times since the problems started occuring. No applications that would cause this has been on while in game (only origin and browser in the battlelog). The games are on the HDD

It's not the SSD because i've tried using windows on the program HDD and it only got worse
