MSI GTX 960 Driver Issue

Recently I got the MSI GTX 960 2GB OC. I'm have problem with its drivers, no matter what os I use (tried Windows 7, 8.1 and 10) once the drivers are installed, the drivers are getting updated and installed again and again when I run Nvidia Geforce software (the driver software came with the GPU).

The GPU is not detected by the Direct X Diagnostic tool (DXdiag), and the Device manager has a yellow triangle on it Properties say "Windows has stopped this device because it has reported problems (Code 43)".

Configuration:
CPU: i5 6402 P
GPU: GTX 960 2GB OC
Mobo: MSI B150M Pro- DH
RAM: 8GB GSkills Rip Jaw DDR4 2400
PSU: Antec 500 W
HDD: WD 1 TB Caviar Blue

Solution
Is the gtx960 used or new?
Do you try to clear the CMOS?
Do you test the gtx960 in other PC? Or other GPU in this PC?
I think either the PCIe slot or gtx960 may has problem. By the way check the device manager again to see what yellow triangle you got now.

Sorry, I might sound a bit dumb, but should I follow your steps exactly the way you've quoted or should I install a fresh OS and then follow the steps sequentially?

The GPU was defective, it was a new card, so had it replaced. Working fine now. Thanks for all your help. Appreciate it. :)
