Screen splitting & vertical lines

Hi all,

I have a problem with my screen display. Occasionally, it would split the screen and there would be vertical lines on my screen.

My Specs:
I5-3470 @ 3.2Ghz
AMD R6850 Cyclone 1GD5 Power Edition/OC

I have attempted to reinstall my drivers but the issue still persists. Can anyone provide a solution the problem? Thanks.

photo 2 and 4 are signs of gpu memory failure or the gpu failing. what i would do is turn the pc off. make sure the card locked in. then i would see if it still under warranty. some gpu do have lifetime warrantyies. would try and rma it if it out of warranty i would replace it.
