Galaxy Note 5 Keeps Refreshing?

So I own a Samsung Galaxy Note 5 that I've had since about Thanksgiving 2015. Just recently it has started "refreshing" for lack of a better term.

For example; I'll be on Instagram, scrolling through all the selfies, and suddenly the app will refresh, taking me back to the top of the page. Facebook does virtually the same thing. Snapchat will close out of whatever I have (so if I just took a pic, it will close it) and go back to the camera main screen which stays black for about half a second. Youtube will do this too (and to be honest I think just about every app using network will I just haven't noticed) but when Youtube does it, the video will pause, it will rebuffer the part I'm on, and show the thumbnail and I have to press play again.

I'm not sure quite what causes this, or how to fix it. It definitely isn't a regular interval either. I also don't think it's my WiFi connection, because this actually started when I was on vacation, using my cabin's WiFi so it isn't just at home. I also believe it has done it on data, but I'm not for sure. What is going on?

I know that mobile phone is able to keep refreshing when you have got a bad app or game, damaged phone memory card, corrupted phone operating system, virus infection or the like causes. You may need to check these elements one by one to take chances:
1). Remove or uninstall the app or game that let your phone refresh
2). Check the used phone memory card on a CP to see whether it is workable.
3). Reinstall or update phone OS.
4). Do antivirus scan there.
Overall, hope these tips can help you anyway.
