Specs:
GPU - Gigabyte Nvidia GeForce GTX 780 GHz Edition
CPU - Intel Quad-core i5 4670k @ 3.4GHz
Motherboard - Gigabyte Z87-HD3
RAM - Corsair Vengeance LP 8GB (2 x 4GB)
PSU - Corsair CX 750W
HDD - Hitachi HDS721010CLA332
SSD 1 (OS + games) - Crucial BX100 250GB
SSD 2 (games) - Samsung 830 Evo 250GB

Please note that although I set the category to 'graphics cards', I'm not certain about where the problem arises from, but it can't be due to software because the problem has been occurring over several windows installations (even different versions of Windows - 7 and 10) and over nearly two years (since I purchased my GPU). Ebuyer had apparently repaired my GPU when I sent it to them about a year ago [I also sent it to them soon after I purchased it, which was even earlier, but they said they 'couldn't find any faults' apparently].

When I play an intensive game and my GPU reaches around 70C, a loud buzzing noise (click here to listen - https://clyp.it/pyxb55zo) continually comes through my speakers (or headphones), the game and my computer completely stop responding, and the screen freezes on the frame in the game when the crash occurred. I can't do anything except turn off my computer via the power button.

Here is an image which shows some hopefully useful information to help diagnose the problem:

I would greatly appreciate help to resolve this problem.
