Specs

Processor - amd phenom 2 X2
Mb- asus m4a785td-v Evo
400W psu
Stock heat sink which is good

First the graphics card isn't the problem the problem exists when I plug the vga cable on the motherboard port

Earlier I had problem when I switched on the pc it didn't boot while the fans were on after trying for 4-5 times it finally boots
This happened for a week after that when I used continuously for 1 hour
The display simply goes to lines randomly

The psu is 4ooW

About a month ago I had the same problem with my other pc which Is core i3 I am using the psu of that build in this one

I am attaching a pic of the screen and voltage monitor from bios

You need to specify the make and model of your PSU. There area boat load of crappy PSU's in a sea of reliable unit, so which one is yours? If you can identify it, please shoot us a picture, we'll help in identifying it. Your system's specs are also partial, can you please pass on your full system's specs and list them as:
CPU:
Mobo:
Ram:
PSU:
GPU:
SSD/HDD:
Case:
Monitor:

The artifacting(squiggly lines) on your screen is an indication that the GPU doesn't get the necessary power or the iGPU is overheating it can also be a sign that it's been damaged.
