Graphics Card Sometimes Losing Connection Under Heavy Video Load

Recently my graphics card, or what I believe to be my graphics card, has been losing connection to my monitors (two of them so I don't believe it is a monitor problem) when under a load of videos. So far, it has only crashed when I am watching twitch, sometimes when only one stream, but mostly when I have two streams open. It has only crashed once when I was watching YouTube. When doing this, I am only using max 60% of my total GPU capability. I have played GTA V multiple times at ultra graphics without my monitors losing connection to the card. I have not cleaned by dust filters in about 2 months, so it could be that.

RAM - Corsair Vengeance 16gb
GPU - Sapphire radeon hd 7950
PSU - EVGA 600b Bronze 600w
Motherboard - ASRock h97m pro4
CPU - Intel 4590


Edit - Once it loses connection, it usually regains connection to the monitors after about 6 seconds.

it cannot be ur monitor...and if both cards are doing the same, chances are, it aint the gpu as well...u have to look elsewhere...
start by cleaning up the pc and resetting the cmos and/or jumper settings...also change the cables...
if that doesnt work out, then u ned to test with another psu...
