I build this computer a month ago and it runs smooth as hell, but after I saw what HWMonitor shows and after some research i learn that the +12v rail should never ever reach 15v what so ever but somehow my psu didn't give me any trouble like blue screen yet but I'm really concern cause I don't want to break my psu or burn my other components..... please help!

Software reads the voltage not at its destination but from another part of the motherboard. It is not always inaccurate, and really there is no reason to be looking at any software voltages in the first place. It is just a waste of time. If the 12V rail was really at 15V I'm pretty sure you would easily have damaged hardware by now. I would like to say overvoltage protection would kick in but unfortunately power supplies' overvoltage protection kicks in usually so late that its purpose is often left in the dust.
