My SSD can't possibly be failing...right?

So I have no idea what is going on with my PC lately. The way it's behaving it seems like a failing hard drive but this seems unlikely given the age of my build. If anyone has any insight into what might be wrong I'd be very grateful. Anyway here are my specs and the problems I've been having.

MSI Z170 Gaming Pro Carbon Motherboard
Intel I7 6700k CPU
8GB DDR4 2800mhz RAM
AMD R9 380 GPU
240GB Intel 535 SSD
1TB Seagate Barracuda HDD

All of this started about a month ago. My PC would randomly slow down. File explorer would be very slow when opening folders. Sometimes I would know it's happening because my Plex videos would take forever to buffer and when I went into Plex and refreshed it would go through each file very slowly as opposed to scanning everything in a couple seconds. Rebooting would solve this issue and my PC would be back to running fast. Eventually I ran sfc /scannow which said it found corrupted files and repaired them and that seemed to fix the problem for a week or 2. Then, last night while I was playing Overwatch, my game froze. I alt tabbed and tried to close it but nothing was happening. Right clicking anything did nothing. Ctrl alt del did nothing. I had no choice but to reset by PC. However, instead of booting into windows it went to a black screen that said Please insert boot device and restart. I rebooted several times into this screen until finally it booted back into Windows. Everything was agonizingly slow, however. On par with my celeron laptop from 2008. It took a while, but was able to open the Intel SSD utility and ran a full diagnostic scan overnight. I also did an error check on my HDD. When I woke up neither tests came up with any problems and my PC was running like normal. I built this PC in March so I don't think my storage drives are failing, also the Intel utility says my SSD is at 100% health.

Well it's happening again but this time it's not resolving on it's own. My SSD isn't recognized in the BIOS. It just says please insert boot device and restart. Sometimes if I unplug the SATA cables and plug them back in I am able to boot into Windows but that's about it. Once I log in I can't do anything. I can move my mouse around but that's it. Clicking on things does nothing. Before all this the Intel utility still said my drive was at 100% health but that can't be the case. I plan on buying a Samsung 850 Pro to replace the Intel SSD. This has to be a bad drive right? It doesn't seem like it could be anything else.
