How to claim $30 from Nvidia Gtx 970 lawsuit?

Haven't found any recent articles explaining how exactly to claim the $30 refund from Nvidia's false advertising lawsuit settlement. I bought my EVGA Gtx 970 (Fallout 4 Edition) from Best Buy less than a year ago and when I called them they said they had no knowledge of the suit and couldn't help me.
So do i contact Nvidia directly or EVGA or is there a website or something i go to?

Does anyone know where I can go to get this rebate, or find more info about whether or not the court approved the settlement? It's after the 24th but I can't find any news on this. All of a sudden I can only find articles from 2015 or early 2016.

Nice. Hopefully these scammers pay up. Because I'm a repeat customer already (gtx 1070) even after this scam.
