This is a simple one, I'm just wondering if AMD A4-5300 APU 3.4Ghz Processor paired with a ZOTAC GeForce GTX 960 2GB GDDR5 PCI Express and 16gb ram be able to handle modern titles?

I stress this PC will be for gaming, and not much else.

If not, what else would I need?

Here are the specs I'm attempting to upgrade to 2016 standard. Medium at 30 or so fps would be fine with me..

Any of the quad core FM2 Athlons would be a good upgrade. The 750k or 760k like mentioned about would work. You could get an 860k or 880k if you're BIOS is up to date.
An Athlon x4 750k or 760k would be good choices.

AMD CPU cores share resources, and are slightly less than full cores. When both of your CPU's cores are loaded up, each one only performs at 80-90% speed due to having to share with the other core, making your CPU slightly less than a true dual core in an era when many games don't run well on anything less than a quad. All FM2 and FM2+ CPUs are considered pretty low-end today, but a 750k or 760k would still be a sizable improvement over what you have.

The degree to which your CPU holds you back will vary from game to game. There would be places in a game like Guild Wars 2 or Arma where you won't get more than 15-20fps regardless of what GPU you have - you could put in a pair of GTX 1080's in SLI and you'd still have terrible framerates. Other games will run smoothly. What do you typically play?

Typically I play single player games, - I'm looking to bulk my rig up to play AAA single player titles, The Witcher 3, No man's sky, Deus Ex, etc.
I understand it's a hefty challenge to take what I have to being capable with those games.

What upgrades would you guys suggest?
