Graphics Drivers Crashing

Hello there
I have N760 MSI Hawk edition card and from last few months i have been getting this error
"Display driver stopped responding and has recovered." and then
"Application BF4.EXE has been blocked from accessing graphic hardware." (Can be any game not just bf4)

I can not find what is causing this problem , totally annoying.

What i have tried so far is

1. Updated drivers to latest , rolled back to 2 to 3 version of drivers with "clean installation"
2. Reinstalled windows 7 two times , installed windows 10.
3. Did same GPU drivers reinstall 3 times to older and newer
4. OC'ed gpu by 10 to 50MHz , that started crashing my game ever faster and recently than normal clocks.
5. Down-clocked by 16 , this option kind of reduced the chances for crashing. for example if my bf4 was crashing in 10mins after starting in normal clock settings then in down-clocked mode it took 20 to 25mins to crash. late or sooner , it did crash though.

Also i noticed , after my drivers and game is crashed, If i dont restart my pc and continue playing games then Core clock wont go above 524Mhz , its kind of locked there. I have to restart pc to get back to normal clocking and its the same issue again after starting game and loop continues.

Also the GPU usage during playing games fluctuates from 50 to 90% which means the GPU isnt giving its all , i literally get fps drops in game. but when GPU goes around 95% usage it will crash as described above.

guide me through this please , i am so tired of finding solution for this. its been more than 2 weeks i got nothing about this. Everything i found on internet is either not working or not related to my problem.

Ok, so yea a 970 would be fine to try. Was there any software or hardware changes around the time the issue(s) started?

If the 970 works fine in your system and is boosting correctly then the issue must be the 760.

All i can do is check a 970 on this system , cant manage a psu. still i think by testing 970 it will atleast make one thing sure if its gpu problem or psu .

thing is i have been using this system from more than a year and it was working well. Psu meets the power requirement for the gpu.
this problem started recently
