GPU clock is stuck at 135Mhz

Hi! I'm using a EVGA GTX 960 SSC. in Precision X, GPU Clock is stuck at 135Mhz. I tried reinstalling the drivers, disabling the driver and enabling through GPU-Z as well as through Device Manager, tried installing an older version, change power management setting under 3d settings in the control panel. none of them worked. please help..:(

Intel G3240
8GB RAM
Asus h81M-C mobo

tried with afterburner. still the same.

check out the screenshot of GPU-Z. TDP is more that 200%. is this normal

is this the one you were looking for. sorry im kinda new the GPU-Z. I was supposed to keep an eye on bus interface rite? it didn't change once I started the test. I kept the loop running for about 10 minutes.

Hi I updated both bios, mobo and gpu. but still no luck.. think theres something wrong with the graphics card. anywy thanks for the help. really appreciate it

GPUz throttling reason is PWR, for power.

Strange quesiton.

Is the card plugged into the auxiliary power connectors?

Also, is windows in power saving power plan?

Perfcap reason explanations:
vRel = Reliability. Indicating perf is limited by reliability voltage.
VOp = Operating. Indicating perf. is limited by max operating voltage.
Pwr = Power. Indicating perf. is limited by total power limit.
Thrm = Thermal. Indicating perf. is limited by temperature limit.
Util = Utilization. Indicating perf. is limited by GPU utilization.
