Should I replace the PSU? Boot issues.

My PSU is "Ultra LSP550 550-Watt Power Supply - ATX, SATA-Ready, SLI-Ready, 135mm Fan " and the PC is about 5-6 years old.

The issues are as follow:

#1: The PC from time to time (maybe 5% of the time) takes 5-10 minutes to wake up from sleep showing a blinking green light during the waiting time.

#2: My USB devices will spontaneously eject and insert, sometimes every couple of hours, sometimes endlessly until I unplug them.

#3: After adding a new SATA drive into the PC it would not boot or even POST until I removed every piece of hardware from the MB and re-inserted it. (I made sure everything was firmly seated before doing that, it didn't help)

#4:After running memorytest for 3.5 passes with 0 errors I restarted the PC and it ran at a crawl and crashed leaving me in the same situation as 4. It would not POST even after 45 minutes of "booting". Again every connection was removed and re-attached to MB before it booted again.

#5: This morning, 12 hours after "fixing" the issue again the computer did the #1 thing again and took 7 minutes to wake from sleep.

The PC basically works perfectly fine when it isn't struggling to boot.
What do you guys think?

Your voltages are really low and bellow the miniumum recommended so i think you need a new power supply and also your cpu temperature is really high if you overclocked it you need to reduce overclock or get a better cooler,or if you have a lot dust in it than just clean the dust and check temperatures after you do that.
