Very Slow HDD

I have a fairly old AIO PC that i got from my, she has had it forever, its been pretty fast, i want to keep the current HDD, but even just running at the desktop nothing else and a fresh install of windows 7 the disk usage is at 100%. Any ideas, none of my other computers are like this, do i really have to replace my hard drive or is there a fix for this?

See if it does it in safe mode. If it is okay in safe mode then you can be pretty sure there is a process or service that is running and causing the issue.
A good utility for checking SMART status is CrystalDiskInfo, per jsmithepa it is a good idea to check if the drive is reporting errors before starting to swap parts.
