I have a fairly old AIO PC that i got from my, she has had it forever, its been pretty fast, i want to keep the current HDD, but even just running at the desktop nothing else and a fresh install of windows 7 the disk usage is at 100%. Any ideas, none of my other computers are like this, do i really have to replace my hard drive or is there a fix for this?

It may be a good idea to back up your data first.
After that, as just jsmithepa suggested, you can test the drive with some of these: http://www.tomshardware.co.uk/forum/282651-32-best-diagnostic-testing-utility
Also, you can go to Resource Monitor and see what is causing this.

See if it does it in safe mode. If it is okay in safe mode then you can be pretty sure there is a process or service that is running and causing the issue.
A good utility for checking SMART status is CrystalDiskInfo, per jsmithepa it is a good idea to check if the drive is reporting errors before starting to swap parts.
