What does "Caution" status in Crystal disk info means?

I was getting frequent startup problems, corrupted BCD and CHKDSK stucking in the process. I checked the disk with Crystal Disk Info and got Caution as the status. What does that mean?
Is my hardisk failing for sure?

I have a Lenovo G50-45 AMD-A8 laptop with Toshiba internal 500GB HDD. I tried installing windows 10, 8, 7 and getting startup problems in all of them. Check the screen shot below for more information.

Solution
It means your drive is probably dying.

Reallocated sectors
Uncorrectable sectors

It is dying.
Back up your critical info on that drive, and prepare to replace it.
Hi there visheshsaxena,

I would totally agree with USAFRet. The drive has some weak and some bad, that were already reallocated, sectors. It could become unresponsive at any time, so you should really back up the data stored on it.

If it is under warranty, then I guess you can just contact the place you got it from.
