OS: Windows 10 64bit
Drivers: Radeon Software Crimson Edition 16.9.1

Describe your issue:
Provide a detailed description of the issue you are experiencing and the steps required to reproduce the issue.

Issue has only happened during gaming/stress tests. The screen turns black with the occasional sound stutter, when/if it returns, the screen remains at 1440p but changes to 60hz. At 60hz, the black screen error seems to go away.
Details about applications/games affected.
Furmark: Doing the 1440p stress, screen flicks for a second at a time until it stays black and returns to 60hz. Tests never show black screen at this refresh rate. If I stop the test and turn the monitor off and then on again, screen goes back to 144hz.
Will occasionally AMD notification error of: the system has detected a link failure and cannot set the requested resolution and refresh rate on your displayport display.

Counter-strike:Global offensive (running at 1680x1050 resolution scaled full panel).
Scenario 1: screen turns black for about 1 second then returns- stays at 144hz.
Scenario 2: screen turns black and sound stutter/loop begins. I remedied this a few times by turning the monitor off/on.
Scenario 3: screen turns black and comes back at 60hz.
Scenario 4: screen turns black and stays black, when I alt-tab, sound stutter and screen flicker begins.

GTA5 (1440p)
Game will not change to 144hz in fullscreen. When I attempt to change the settings, the game will become windowed mode. Will do fullscreen at 60hz.

Things I have tried is changing the pci-e cable, clean installs, using the recommended DP1.2 verified Accell cable (3m/10ft).

Does anybody know the problem? How can I get it to stay at 144hz stably?

That sucks, I bought another cable (startech verified dp1.2) just to make sure it wasn't a cable issue. If that doesn't work then I'll try to get a replacement. Thanks!
