Android 6.0 locked my micro SD card, how do i format it?

short version is: got a 64gb micro sd card that i cant format, windows says its write protected, Diskpart says "the request could not be performed because of an I/O device error.".


so i had this micro sd card as part of "internal storage" on my HTC One M8 and it had been working fine until i started getting random app crashes, and then finally the home launcher stopped working (on an endless loop), so i factory reset it, and as expected, the formatted sd card stopped working, because it only works on that particular android and once that was gone so was the sd card.

lost a few thousand pictures but whatever, i just want the storage back, android can't format it, and windows is giving me those errors.

any idea what i have to do? windows says its in a "RAW" format, and that it's write protected. i'm out of ideas.
