What kind of graphics card is this?

I bought this two years ago. I have lost the box and I can't find it on the PNY website. I want to identify it to get its specifications as I want to get a new one.

So I assume it doesn't work? Have you tried looking up the serial number? If it doesn't work there would be no harm in removing the shroud and heatsink to look at the actual chip underneath. You would need to clean off the thermal paste in order to see it. It would have a code name, such as GM107 for Nvidia's Maxwell GTX 750 Ti or something else.

I looked around online, and it must be a 750 Ti. PNY doesn't offer a GTX 750 with a dual-fan solution, and doesn't offer anything else in the 600, 700, or 900 Series that looks like that. It must have been one of the first cards, too, because it doesn't have the "GTX 750 Ti" badge near the fans, or the black DVI casing.
