I'm looking to upgrade the graphic card on my PC. I do some amateur 3D modelling and I play League of Legends a lot, but I keep getting some slight lag on the game even at lowest quality. I do not know anything about all of this so I will just copy here the info that I find. Note: the computer is one of those slim office style computer.

Windows 8.1 professional 64 bits (6.3, version 9600)
Intel Pentium D CPU 3.40 GHz (2 CPUs)
6144MB RAM
BIOS: Pheonix ROM BIOS PLUS Version 1.20 2.4.1
Sapphire Ati Radeon Hd 4350 512 Ddr2 Pci-e X16 Hdmi Video Card, Sku#: 11142-07
Dell PSU Model: d280p-00 (there's a bunch of info written on the label so here they are)
Input 100-120V - 6A, 200-240V - 3A,
47Hz-63Hz
Output +12V / 15A, -12V / 0.5 A
MAX POWER 280W +5V / 15A, +5VSB / 4A
MAX COMBINED POWER ON +5V & +3.3V OUTPUT IS 108V
+5V & +3.3V 108W
+5V & +3.3V 108W
DATE CODE: 0728
This is the info of the motherboard:
Manufacturer Dell Inc.
Product 0MM599
SerialNumber ..CN1374076M06DQ.

My goal would be to be able to play League of Legends on at least medium quality without lag.

With a tiny +12V rail rated for only 15A, I wouldn't go much more than the GT 730 DDR5 64 Bit.
If this is what the insides of your Optiplex 745 SFF looks like, you have another issue. The gfx card must be single slot and very narrow. And obviously, low-profile.

One of these two GT 730 low-profile cards should fit: http://www.newegg.com/Product/ProductList.aspx?Submit=ENE&IsNodeId=1&N=100007709%20600519936%20600419828%20600038363%20600038364%20600007797%20600007799

Gaming depends mostly on the GPU and the CPU. You are hurting on both counts. And having a slim line case will limit your choices of upgrading the gfx card to a low profile design. If possible at all. The tiny PSU is another limitation. The items I highlighted above pertaining to the PSU (power supply) are its weak spots for a better gfx card. What is the exact make/model of the PC you are using? Apparently it is a Dell...
