SDHC Card from Android 6.0.1 Phone is not reading in PC

Hello,
I got my problem with my SDHC Card (Samsung 32 GB). I formatted my SDHC in android as internal storage, I put it out, inserted it in SD Adapter and then in my PC. My pc is not reading it, but adapter and drivers are working fine. It will be readable only if I'm gonna format it via MiniTool Partition Wizard. When I'm in MiniTool Partition Wizard, its shows that its divided to 2 volumes. First volume is 16MB (created by android I think) and the other one is the rest of the sd card. If im gonna delete that 16MB volume or the rest, android will not recognize as the old one. The point I want to do with SD card is that I want to patriot it to 2 volumes which one will be Data and other one will be Apps. After that I want to work with Link2SD but I cant. When Im putting sd card back in the phone its says that its new sd card and not the old one. I didint copy original files earlier because It's not reading if its not formatted by MiniTool Partition Wizard.

Plug phone into computer. Let it install any software it needs to recognize the phone and, subsequently, SDHC card inside it. At this point it is probably going to update any software related to the phone and/or the computer using the phone. Once you are sure everything is up to date try putting some photos or music onto the SDHC card from the computer. Any further advances in use are going to stem from the computer and phone at least establishing some dialogue. So start here and we can work outwards.

Ok, I plugged in, on my phone I picked File Transfer and I put some music on SDHC. So what next? Do I have to try now put out sdhc and put it to PC?

No, just repeat exactly what you did to get everything you need off your computer on it and set up the rest on the phone. If you found a solution that works why fight to make another one needlessly.
