Display Driver crashing

Hello

Over a month Ive had problems with my PC (Specifically my graphics card I think), constantly getting errors such as

"Display driver amdkmdap stopped responding and has successfully recovered."
Where my monitor loses the picture completely, it just goes black for a few seconds before returning back to normal..... Sometimes it will only repeat itself once or twice, other times once it begins crashing it wont stop so would consistently keep going off/on/off/on until I force restart.

Other problems I have encountered are playing games where that game application will lose display just in that applications window, it will infact turn a strange grey colour and then I will receive the error message
"Application *** has been blocked from accessing graphics hardware"

And in other cases my entire system has completely locked up, cant even ctrl alt del, I am forced to restart.

What have I done? Everything. You name it, I've done it.
100x DDU reinstalls, to latest drivers and some abit earlier.
Formatted my PC
Researched my cards stock core speeds prior to manufacturers overclock, returning those speeds to stock..
I am really having no joy now

I wanted to show a picture of my speeds and setup in MSI Afterburner to see if I could improve anything thats more stable, I am thinking more the voltage but it already looks like it has been increased as its not set as 0, though this wasnt me. All I have moved is the power limit and the core speed.
Please see:

Many thanks
Lee

No I have no spare PSU.. Is there any way I could test without using another PSU?
I have had this PC with same PSU since 2011 though, upgraded the graphics card in 2014 to the R9 290.
It was fine for 2 years, nothing has changed so if its the PSU why now?

Why in MSI Afterburner does it say my Core Voltage is +25 ?
I didnt change that, my card isnt even overclocked so surely that core voltage should be set as 0 ????? Anything higher is going to use more power..
I could maybe start with that.

Also removed the overclock of my CPU from 42x back down to 33x which is the default.

So those 2 changes, should now be using less power. Will see how it goes
