Troubleshooting freeze/crash issues related to hardware - possibly Mobo or PSU

For several weeks now I have been troubleshooting an issue with my machine, and have reached the point to where I believe it to be hardware related, specifically either the PSU or MOBO.

Specifications:
Corsair RM750 PSU
MSI Z97 Gaming 5 motherboard
i7 4790K processor
Corsair Vengeance 8GB x 2 RAM
840 PRO 128GB x 2 SSD
1TB x 1 HDD
AMD RX480 GPU
Corsair K95 and M65 peripherals
Fiio E10K DAC
Windows 10 Professional

Issue:
System freezes/halts randomly

Symptoms:
System completely freezes
KB and mouse cease working - cursor usually dissapears, and the Caps Lock/Num Lock no longer cause the LED indicator on the KB to cycle when pressed
If sound was playing, it will become severely distorted and will continue until power is removed from the machine
Video is not affected - whatever was on screen will be frozen in place until power is cycled
Sometimes there is a BSOD, but it almost never generates an error in event viewer. The BSOD screen actually freezes.
Even when it does generate an error, it is usually always something different and not consistent - memory error this time, video driver the next, and then a kernel error after that

When does it occur:
When the machine is sitting idle
When the machine is under full load
When working, playing a game, watching a video, etc
On the login screen for Windows
During the Windows logo screen during boot
While actually in BIOS

Steps taken in troubleshooting:
All temps are low and stable
Reinstalled Windows 7/8.1/10 fresh copy
Removed and reinstalled all drivers
Update BIOS to more recent version
Disabled all overclocking
Disconnected and reseated all components (connectors, RAM, GPU, HDD, SSD)
Tried 1 stick of RAM
Moved RAM from 1/3 to 2/4 position
Tried a new GPU
Tried a new HDD
Tried a new PCI bluetooth/wireless card
Tried using different SATA ports
Tried using different peripherals (KB, mouse, unplugged hub, unplugged USB DAC for headphones)
Tried using different monitor

OK, so now with all that done, I've been monitoring the PSU voltages through HWinfo64, and was able to catch the +5V rail momentarily drop to 0V, as can be seen in the screenshot. No other anomalies have been seen. This leads me to either the PSU is failing, or the MOBO is failing.

Any other ideas or recommendations based on what I have done so far is appreciated.
