PC boots to black screen with white underscore from SSD. Is my SSD dead?

The SSD card is about 4 years old for reference.
When I boot, it boots to a black screen with a blinking white underscore. So I thought that maybe I was booting from the wrong drive, so I went into my bios menu and it gave me the option to boot from my HDD or SSD, so I picked SSD(which my Windows is installed on) only to find the black screen again. I repeated for my HDD just in case. Both failed.

So I went into the menu again, and found that when i go to boot priority, my SSD wasn't even on the list of available. I'm wondering with all these hints, my SSD is just dead.

CPU: i7 3770k
GPU: R9 290
Motherboard: AsRock z77 pro 4
HDD: Hitachi something 1T
SSD: Sandisk 128gb
