Severe lag on my PC... can't figure it out.

About a month ago I started to experience some pretty troubling issues with my PC... it happened suddenly when I installed/began playing Call of Duty 4. My PC would slow down to a crawl--the frame-rates would dip to about 5fps, and I could barely move my mouse around on the screen. After this I tried to restart my PC to see if that would remedy the issue and it did, but would return and continue AFTER I played some graphically intensive game. At times the screen would go black and then return, revealing a "caution" symbol on my taskbar (but whenever I tried to click on it it disappeared).

I did a few things...
- Ran Memtest, there were no errors found.
- Re-applied thermal paste to my CPU.
- Tried running on Integrated Graphics to test if the issue was the video card, but found that the stuttering/slowdown was still present.

When the PC begins to slow the CPU load jumps around drastically (as does the hard disk load). I checked in Task Manager and I could not see anything that had more than a 20% load, which baffled me.
I don't know the issue and I would really like some help. Any suggestions?

1| You should run memtest86 for at least 10 passes before ruling out the ram.

2| When posting a thread of troubleshooting nature, please be sure to pass on your full system's specs inclusive of your OS. List them as follows:
CPU:
mobo:
Ram:
GPU:
SSD/HDD:
PSU:
Chassis:
OS:
Peripherals:
If possible, audio equipment.

3| Might I ask where you got your copy of CoD 4?

4| Have you made sure your motherboard BIOS as well as your chipset and GPU drivers are up to date? If you're on the latest GPU drivers uninstall and revert to last known stable driver revision. Using DDU will help with the existing GPU driver's removal.
