Gpu crashing, gpu fans spin up, black screen

Hi guys, as the title suggest, my gigabyte r9 fury is crashing at stock clocks and voltages, the fans spin up to 100%, both my monitors go black and the system has to be hard shutdown. This is the second r9 fury card that has had a similar issue in my system (RMA'd the first one).
Ive already checked the cards temps, maxes out at 40c, cpu temps maxed at 65c at max load

System specs:
Msi gaming 3 z97 mobo
Intel i5-4690k
Trident 1866mhz 2x4gb ram
128gb m.2 ssd
2tb wd green hdd
Corsair cs 550w gold modular psu
Gigabyte r9 fury windforce 3x
Corsair h100i
Asus dxg sound card

Things ive tried:
1. Full driver flush using ddu, tested older and beta drivers
2. Updated motherboard bios
3. Reset motherboard bios and cpu overclock settings
4. Tested a gtx 770 and an r9 290 and cannot reproduce these issues
5. Tried secondary gpu bios
6. Underclocked and mildly overvolted the card, system appears stable at core clock 700mhz

Any ideas or suggestions are greatly appreciated

Have you tried a psu swap? Your corsair is a tier 4, so they are known to have unstable 12v rails under high load. Your build would be using just under 500w, and normally you want at least 30% buffer to keep the volts stable, so I would be looking at a 650w psu tier 3 or above.
