Troubleshooting black screen, help with voltage readings

When playing games or under load, my screen suddenly goes black. I can game for maybe an hour before it goes black. The system doesn't crash, the fans and lights still run, but I can't bring the display back up without restarting.

GPU/CPU temps and fan speeds are within acceptable levels, case is clean inside
Reinstalled drivers for GPU from scratch
Reseated power cable for GPU
Tried multiple monitors with same result
Reset BIOS to default

Intel Core i7 5820K 3.3GHz
ASUS X99-A USB 3.1 (Intel X99 Chipset)
16GB DDR4 2666MHz
750W EVGA SuperNOVA
1x GeForce GTX 980 Ti 6GB

I'm trying to test my PSU now with SpeedFan, but honestly I don't know what I'm looking at. Could someone tell me what I'm seeing for the voltages down at the bottom, and if they're within acceptable levels?

well,that's a hard question to answer.it could be your gpu or memory stick or motherboard or CPU or even a simple software related problem!
I had a similar problem( not exactly the same),my system used to shut down or go black screen during load or light use( light use:like internet browsing or exiting a game,saving a game).as a friend here...

It is not reliable to use software monitoring and speedfan voltages I see there are not understandable.use a multi meter to measure voltage at load(only if you know how to).I guess you have not overclocked the card,right?

The CPU was overclocked, but I reset the BIOS to defaults. I'm not familiar with using a multi meter, and I don't own one. Besides the PSU, what else could it be?

I had a similar problem( not exactly the same),my system used to shut down or go black screen during load or light use( light use:like internet browsing or exiting a game,saving a game).as a friend here suggested,I started taking out memory sticks and guess what ,no problem after taking one out. I am still testing to make sure my problem is gone and most probably it is.your problem always occurs at load so I suspect your PSU or GPU.test your cpu with prime95 please.if it passes for about 2 hours there is even less possibility of it being faulty.(a bad PSU can result in system crash when you are testing with prime too but the possibility is lower because your GPUs are not working hard then).try running system with one memory stick in different memory sockets at a time to see if the problem is gone.
I personally would still test with another GPU or PSU as a first step.
BTW I meant over clocking your gpu not cpu.
