GTX 950 Performance Issues

I've had this card for less than a year, not using it very much. Any time I watch HD video, or play ANY game I get horrible fps (2-40) with it never being anywhere near smooth, and the game is unplayable. While playing a game, my CPU will be at less than 20% usage. I've tried uninstalling drivers using the driver removal tool, switched PCIE slots, rebooted PC, and no change. I've also changed the changed PCIE power mode to maximum performance in windows.

I've tried playing BF1 on the LOWEST settings and saw no difference in performance.

GTX 950
i5 4690K
250GB SSD
650W coolermaster PSU
16GB RAM
Gigabyte GA-Z97MX
Windows 10 64
ASUS 24" 1080p (DVI main monitor)
Acer 27" 4k (display port)

If anyone has suggestions I should try that would be much appreciated.

I ended up fixing the issue. After swapping in another video card and having the same problem, I replaced the PSU and it worked like a charm :) getting ~70fps on high now. What I noticed was the frame rate's were dramatically spiking. I would've though that if the PSU was bad it would either work or it wouldn't. Definitely a strange issue, but thought I'd share what happened.

I tried both of these and unfortunately my fps is still in the red on BF1 on lowest settings.

That's good to hear! You're right about a failing power supply causing performance issues. Happen to me back in 2007 playing Obivion, but I'd forgotten about it.
