Can't get 4k 60Hz on a Samsung 40KU6400 UHD tv from a Quadro K4200 card

Need badly some help here!
I have a Samsung UE40KU6400 UHD tv, a Nvidia Quadro K4200 and a club 3d displayport 1.2 to HDMI 2.0 adapter plus a HDMI 2.0 cable all connected to give me 3840X2160 at 60Hz as it supposed to.
This Samsung model only gives me the possibility to control UHD color setting on HDMI 1 input and the K4200 card have 2 DP 1.2 ports and 1 DVI output. So I've started to connect the tv through the DVI card output (DVI to HDMI adapter) to the HDMI 2 input. All's well here, it gives the expected 3840X2160 30Hz. Then I switch to DP 1.2 out from card (DP 1.2 to HDMI 2.0 adapter + HDMI 2.0 cable) to HDMI 1 input of tv with UHD color set to on. And the problems start. The tv went on a 2 blue bars flickery screen. I tried everything I can possible remember to revert this but nothing seems to work.
After that I reconnect the DVI/HDMI adapter into HDMI 1 and something very strange happens!
My Nvidia control panel says it's in 3840X2160 - 60Hz and it flickers a lot when displayed.
The image displayed seems normal but when I went to the internet to see what resolution I'm having it says that I've only 2560X1440 and the most odd thing is that the card it's supposed to do 3840X2160 60Hz but only through the display port, not through DVI.
What could it be wrong here?

OK, I'm not going to be much help with the card. However, I have a Samsung UE40HU6900 and can say it was 'very picky' on the type of connection, the cable and which port on the TV it was plugged into.

I've just installed a Palit GTX1050 Ti, to get my 4K at 60hz, and it took a while to find the right connection and cable combo. A HP display port to HDMI cable failed to get me past 1080p, in the end I found a HDMI 1.4 cable, and connected it to the HDMI on the card and HDMI1 on the TV to get it working. It didn't work on HDMI2 or HDMI3 ports on the TV, and I had to go to device setup on the TV to get it to work.

In the end it was worth it and I have 40" at 4K on my desktop, but it took quite some time and playing with connections to...
