Multiple Catastrophic Failures

I have no idea what has happened to my computer as of late. I build my own about a year ago, and it has been smooth sailing ever since.. up until about 2 weeks ago, when my monitor suddenly lost signal, but my machine kept running. A quick restart fixed the issue, but it seemed to be temporary, as it became more and more frequent to a point where I now cannot use it.

There are three problems that I am encountering. All 3 of the failures happen on a daily basis now. I honestly have no idea what part of the system the culprit might be, but I think it may be my GPU.

Failure Option 1: During use (hardware intensive or not) monitor loses signal, and machine keeps running (lights on, can hear the fans) the connections are all solid, and I am using a DVI-I cable. Sometimes a restart fixes it, sometimes it doesn't.

Failure Option 2: During use (hardware intensive usually) monitor loses signal, and the machine keeps running but I hear a recurring bzzzz like a freeze in my headphones.

Failure Option 3: During startup my computer says "please power down and connect the PCIe power cable(s) for this graphics card. Rest assured, all of my cables are in place, and secure. Opening the case and checking again doesn't fix anything. It still shows the error. It seems to be just luck that it will boot, because if I restart it a few times on of those time without changing anything in the hardware, it will boot.

Not Critical Failure: Since building the computer, something has been rattling, and it seems to be coming from the PSU, but when I run it with the case open it don't see it rattling. It sounds like this: http://vocaroo.com/i/s1eb1WeQFass

At this point I run into one of these at least 10 times a day. Any help is greatly appreciated.

Solution
The fact that you sometimes get the "please power down and connect the PCIe power cable(s) for this graphics card" message, points to an issue either with the graphics card or PSU.

I would firstly reseat the graphics card, and try a different PCI-E power cable, plugged into one of the other ports on the PSU (as yours is modular).

If it still doesn't work, just try reinstalling Windows. Make sure you back up your important files though. This is what I would have done.
